Volunteer Coaches Application Forms and Information
The Piscataway Township Department of Recreation values its volunteer coaches and recognizes the vital role they play in the physical and emotional development of our youth sports participants. We thank and commend our volunteers for the interest, sacrifice and commitment they make to our children, our programs and activities as well as the positive impact they have on the overall quality of life in our community. Be advised that all prospective Piscataway Recreation volunteer coaches are subject to meeting the following requirements: • You must complete and submit the Volunteer Coaching Application. • You must read, sign, and submit the Coaches Model Code of Conduct (Twp. Ordinance 03-41). • You must read, sign, and submit the Consent to be Fingerprinted form for the purpose of state and federal criminal background checks according to the terms, conditions and timetable established by the Department of Recreation (Twp. Ordinance 02-14). Please note that electronic signatures are not acceptable on these forms. Please fill out the forms online, then print them, sign them and submit them to the Recreatioin Department at 455 Hoes Lane, Piscataway NJ 08854
